This is Barbara Bixby QVC style code J156485, her Perfume Bottle Necklace featuring a Sterling Silver high-relief perfume bottle with oval Citrines, faceted Blue Topaz top, sea-green Enamel and lots of 18K yellow gold highlights.   It's attached - and removable - to a 16" Deep-Gold Pearl necklace.  These came in two lengths - this is the 16" version.
 
The perfume bottle centerpiece measures 1.4" x .75".  
 
It comes from a non-smoking home in excellent condition, ready to wear.
