This is Barbara Bixby's high-end Couture line sterling silver & 18K yellow gold adjustable necklace, with 18K flowers flanking the center pendant circle.   It has a total physical length, end to end, of 18" with an actual wearable length from 15" at the first connection loop to 17.5" at the furthest loop.  The clasp hook makes connection very easy - no lobster claw to fiddle with.   And as you can see, it features the signature flower dangle at the end of the chain.
